  2. Once you have created an account, an email will be sent to your email address with a validation link. Click the link to validate your account.

The email address must match the current email on file for the PRF.

MMD# must contain the MMD prefix (i.e. MMD00000)

Oregon Id includes oregon Drivers License (DMV)

The Identification Type and Idendification Number must match the current information on file for the PRF.

Important: If you are the PRF for multiple facilities, you will need to create a different user name and password for each.
